Improving Health Services across the Lifespan for Tri-County Residents
The Eastern Adirondack Health Care Network (EAHCN) is a coalition of agencies and organizations spanning the region of Clinton, Essex and Franklin Counties with a mission to improve the health status of our communities, and the access and delivery of health care in the region, through the collaboration of our members.
Our goals include:
* Increase community involvement and locally-generated, innovative solutions to health systems' issues in rural areas;
* Improve the financial viability of rural health care providers in the existing health care economy;
* Increase and strengthen cooperation through affiliations, coalitions, or partnerships to enhance and coordinate an array of necessary rural area health services; and
* Strengthen rural area providers' accountability for improvements in key public health and/or health care outcomes in the communities they serve.
Eastern Adirondack Health Care Network Projects
The Eastern Adirondack Health Care Network (EAHCN) is a coalition of agencies and organizations spanning Clinton, Essex, and Franklin Counties. Our mission is to improve the health status of our communities, and the access and delivery of health care in the region through collaborations among our member agencies.
EAHCN's focus on youth and their families began over a decade ago in Clinton County. Our goal is to assist with establishing collaborative partnerships with organizations in the tri-county area that focus on healthy youth development and prevention of substance abuse, delinquency, violence, and the onset of chronic conditions and mental illness.
Our network's primary strategy incorporates a public health approach to prevention that aims to influence the entire social environment, and integrate prevention activities into the fabric of our rural communities.
